Evaluations

Dr Seikel provides Comprehensive Substance Use Evaluation for medical professionals involved with the Professional Resource Network or the Interventional Project for Nurses. Dr. Seikel requires that all documents and medical records she requests, be received prior to the time of the appointment. This will be arranged through her office nurse. Dr Seikel also offers Comprehensive Pain/Addiction Evaluations, as well as comprehensive assessments for the Federal Aviation Authority (FAA).

• The fee for a Comprehensive Assessment is $500.
